Hemorrhoids vs Anal Fissure: Key Differences

Both conditions can cause bleeding and discomfort, but pain pattern, examination findings and treatment differ.

Pain pattern

Hemorrhoidal bleeding is typically painless unless thrombosis is present. Anal fissure pain is usually sharp and triggered by defecation, sometimes lasting hours afterwards.

Bleeding pattern

Both can produce bright-red blood. Fissure bleeding is usually small in amount and accompanied by pain; hemorrhoidal bleeding can be more prominent and painless.

Treatment differences

Hemorrhoids: lifestyle, office procedures or surgery depending on grade
Fissures: conservative measures first; chronic fissures may need medical or surgical treatment
