Can perianal abscess be treated with antibiotics alone?

Antibiotics alone are usually insufficient. Most abscesses require drainage; antibiotics are an adjunct in selected cases such as systemic infection or immunosuppression.

Perianal Abscess — Patient Information

A perianal abscess is a collection of pus in the tissues around the anus, usually arising from infection of the anal glands. It typically causes increasing pain, swelling and sometimes fever, and generally requires prompt surgical drainage.

Symptoms

Throbbing pain around the anus that worsens over days
A tender, warm swelling near the anal opening
Fever or general malaise in some patients
Spontaneous discharge of pus if the abscess ruptures

Why prompt treatment matters

An untreated abscess can enlarge, spread into surrounding tissues and, in some patients, evolve into an anal fistula. Surgical drainage is usually the primary treatment; antibiotics alone are generally not sufficient.

Relationship with anal fistula

A proportion of patients who have had a perianal abscess later develop an anal fistula. Follow-up after drainage is important so that a developing fistula can be recognised and managed.
