Rectovaginal Fistula — Patient Information
A rectovaginal fistula is an abnormal connection between the rectum and the vagina, allowing stool or gas to pass through the vagina. Causes include obstetric injury, inflammatory bowel disease, prior pelvic surgery, infection or radiotherapy. Treatment is generally surgical and tailored to the cause and anatomy.
Symptoms
- Passage of gas or stool through the vagina
- Recurrent vaginal or urinary tract infections
- Vaginal irritation or discharge
- Discomfort during intercourse
Diagnosis
Diagnosis is based on history and physical examination of the rectum and vagina. Imaging studies such as MRI or examination under anaesthesia may be required to define the tract.
Treatment options
Treatment is individualised. Small fistulas with no infection may be observed initially; persistent or symptomatic fistulas generally require surgery. The technique depends on the location, size and underlying cause.
