Anal Discharge and Odour

    Ongoing moisture, staining of underwear or an unpleasant odour around the anus is a distressing but treatable complaint. The character of the discharge — mucus, pus or stool — points to different causes.

    Common causes

    • Anal fistula with recurrent purulent discharge
    • Prolapsing hemorrhoids producing mucus
    • Anal soiling due to incomplete closure or hygiene difficulty
    • Perianal dermatitis or fungal infection
    • Rectal prolapse in older adults
    • Inflammatory bowel disease involving the perianal region

    Evaluation and management

    Examination identifies external openings, skin changes and sphincter tone. Where a fistula is suspected, imaging such as MRI may be used to map the tract before treatment planning.
